Much of what is written about EMsculpt NEO quotes the manufacturer's "30% fat, 25% muscle" averages and rarely lists who should not have the treatment. This guide is written by the team that runs EMsculpt NEO for Chelsea Clinic and reviewed by our clinician. It gives you the trial figures and the independent reviews, how we price it, the regulator's list of what is cleared and what is not, and the honest answer to who it suits.

What EMsculpt NEO is

EMsculpt NEO is a body treatment made by BTL Industries. A flat applicator is strapped over a muscle group and does two things at once. An electromagnetic field, which BTL calls HIFEM, drives the nerves of the muscle at a rate that a 2019 paper on the original device, by a BTL advisor, describes as "almost 20 thousand pulses" in a 30-minute session. The authors say the resulting contractions cannot be reached voluntarily. At the same time radiofrequency heats the fat layer over the muscle; in small manufacturer-linked human studies the fat reached 42 to 45 degrees, a range at which fat cells are reported to begin programmed cell death (apoptosis), and the body is thought to clear them over the following weeks. The original EMsculpt, which some clinics still run, has the electromagnetic part only and carries no fat-reduction clearance.

Nothing is cut or injected, and you lie down for the session. Two applicators can run at the same time, so two areas, such as both flanks, can be treated in the same 30 minutes.

Does EMsculpt NEO actually work?

It produces measurable changes in the manufacturer-linked trials; whether they are worth it to you is the real question, and the honest version is more useful than the headline.

  • In the only sham-controlled imaging trial we found (Samuels, 2022), 72 people had three weekly sessions or a sham. Three months later the treated group's abdominal fat layer was 28.3% thinner on ultrasound (about 7.6 mm) and the rectus muscle 24.2% thicker (about 2.3 mm), and the change held at six months. The three authors advise the manufacturer.
  • In the manufacturer's single-arm study submitted to the US regulator for the fat clearance, 44 people were enrolled and 42 completed three sessions: abdominal fat down 29.8% on ultrasound at three months and abdominal circumference down 3.2 cm on average, with 88% losing at least 1.5 cm.
  • For the flanks, an MRI study (Cohen, 2024) found fat 28.8% thinner and the oblique muscles 27.2% thicker after four weekly sessions; the manufacturer's single-arm flank study submitted to the US regulator measured a 5 mm average reduction by ultrasound.
  • For the upper arms, fat was 25.5% thinner and the triceps 23.9% thicker in the 25 people scanned at three months (Jacob and Weiss, 2023). For the buttocks, the gluteal muscle thickened by about 25% at three months in an MRI study that recruited 67 people (54 analysed), and a three-person pilot found negligible change in buttock fat.
  • The independent reviews are the sobering part. Three published reviews (Swanson 2023, Kohan 2024 and Swanson 2025) put the average change at about 5.5 mm of fat and 2 mm of muscle across the abdomen in the mostly original-EMsculpt studies, and 8 mm and 5.5 mm across all body areas in the NEO studies. Both Swanson reviews found that almost all the studies were written by the manufacturer's advisors. The 2025 review concluded that the changes were of questionable clinical relevance and that liposuction remains the gold standard.

So the realistic picture is a measurable firming of a treated area and a thinner fat layer, visible at around three months, modest in millimetres however large the percentage sounds, and in our experience strongest in people who are already close to their target weight. Weight does not change in the trials, and the Irish advertising regulator (ASAI, ruling published ) upheld a complaint that a BTL Industries Ireland advert for EMsculpt NEO could create the impression that any user could achieve the pictured results.

What EMsculpt NEO does well:

  • Firming the abdomen and the waistline in someone who exercises and has a soft layer left.
  • Strengthening the abdominal wall after pregnancy; in two small uncontrolled studies of the original EMsculpt the gap between the abdominal muscles narrowed by about 10% to 23%, though an independent review found the change across studies was not statistically significant.
  • Toning the buttocks, thighs and arms, and reducing the fat on the flanks, thighs and upper arms.

What EMsculpt NEO does not do:

  • It does not make you lose weight, and it does not replace exercise or diet.
  • It does not reduce fat on the buttocks or calves: those are muscle-toning indications only.
  • It does not treat loose skin, cellulite or a large, pinchable bulge; a bulge that can be pinched is a job for fat freezing or surgery.

Who EMsculpt NEO suits, and who it does not

In our experience the best candidates are people with a body mass index under about 30, who train or want to, with a soft layer over a muscle they want firmer: the abdomen after children, the flanks, the upper arms, the buttocks. One study of the original EMsculpt found larger changes at a BMI of 18.5 to 24.9; the NEO sham-controlled trial found no link between BMI and result. The manufacturer says the treatment suits people up to a BMI of 35, which was the entry limit of its trial; the US label itself sets a BMI limit of 30 only for the upper arms. The fat clearance covered skin types I to III first and all skin types from 2021 for the abdomen and thighs, with the flanks cleared for types I to IV and the arms for II and III.

The manufacturer's full contraindication list is in its user manual for clinics, which we have not published here. The list below is the one that clinics and distributors relay from the manufacturer's guidance, and we check it against the manual at your consultation:

  • A pacemaker, a defibrillator, a neurostimulator, a drug pump or any implanted electronic device.
  • Metal in or near the treated area, including a copper coil, plates, screws or a joint replacement.
  • Pregnancy, and breastfeeding on some clinic sheets.
  • A hernia in the treated area.
  • Cancer or a malignancy in the area, a fever, epilepsy, a heart condition, a bleeding disorder or blood thinners.
  • Recent surgery, an open wound or an infection in the area. The manufacturer's FDA filing also lists use on injured or otherwise impaired muscles as contraindicated.

Tell us about any of these at the consultation, and about any recent abdominal surgery, a caesarean or a diastasis, which we assess before we treat.

We do not offer Botox or dermal fillers. Our clinic works with what your body does for itself, and EMsculpt NEO fits that: your own muscle grows and, on the manufacturer's account, your own body clears the fat. If you have had injectables elsewhere, we wait two weeks after Botox and four after filler before treating the area.

What a session is like, step by step

  1. Consultation. The clinician looks at the area, pinches the fat layer, asks about your training and your history, and agrees the areas and the number of sessions. You get the plan and the price in writing. Consultations and follow-up reviews are free.
  2. Preparation. In our practice there is no numbing and no gel. The applicator is strapped over the muscle with a belt. Jewellery and anything metal come off.
  3. The 30 minutes. The intensity goes up in steps to the level you can hold. You feel the muscle contract in waves, like a very hard workout you are not doing, with warmth from the radiofrequency. In our experience most people find it intense rather than painful, and it is not nothing; 93% of the people in the manufacturer's FDA study reported none to minimal discomfort.
  4. After. The applicator comes off, the skin is pink and warm for an hour or so, and you go back to your day. Expect the muscle to feel as it does after heavy exercise for a day or two.

How many sessions, when you see the result, how long it lasts

FeatureWhat to expect
Sessions in a course4, about a week apart, which is our protocol (the sham-controlled trial and the manufacturer's FDA abdomen study used three weekly sessions; the flank, arm and buttock studies used four)
First changeMany people feel the muscle working from the first session; in our experience the area starts to look firmer two to four weeks after the last one
Main resultMeasured at three months in most of the NEO studies
How long it lastsHeld at six months in the sham-controlled trial. For the muscle-only version, a 21-patient uncontrolled study by manufacturer advisors measured the changes about a year later: fat about 15% thinner and muscle about 19% thicker than at baseline. In our experience the muscle fades like any training if you stop using it, and the fat holds while your weight holds
MaintenanceNo published schedule. In our experience a single session every three to six months keeps the tone; we review you at three months and decide together

Is three sessions enough? The sham-controlled trial and the manufacturer's FDA abdomen study used three sessions and measured a result, so three is not nothing. The flank, arm and buttock studies used four, which is also our course, and in our experience the fourth is where the firmness becomes visible rather than felt. A single session is a demonstration, not a course.

NEO vs the original EMsculpt. The original has no radiofrequency and no fat clearance. In a 67-person MRI study of the buttocks, NEO thickened the muscle by about 25% against about 16% for the original, and in the manufacturer's comparison the fat change rises from about 19% to about 30% and the muscle change from about 16% to about 25%. If a clinic quotes a low price, ask which machine it is.

NEO vs CoolSculpting. They do different things, and we offer both. CoolSculpting removes fat cells from a fold you can pinch and does nothing for the muscle; it also carries a rare complication, paradoxical fat growth, that EMsculpt NEO has not been reported to cause, though its studies are small. For a pinchable bulge we suggest CoolSculpting; for a soft layer over a slack muscle, EMsculpt NEO; and for some people one after the other. We found no study that has compared them.

NEO vs liposuction. In our view liposuction removes far more fat in one go and can reshape, at the cost of surgery, anaesthesia and weeks of recovery. EMsculpt NEO is for the person with a modest layer who wants tone as well, and no appetite for surgery. If you have litres to lose rather than millimetres, liposuction is the honest answer and we will say so.

NEO vs exercise. The manufacturer's advisors describe the contractions as beyond what you can do voluntarily, but no trial has compared a course of EMsculpt NEO with a training programme, and nothing in the studies suggests it replaces one. In our experience the people who keep their result are the ones who keep training.

Side effects, and the risks nobody writes about

The published studies report mild redness where the applicator sat, lasting an hour or so, and muscle soreness for 24 to 48 hours; some people report their posture and back feeling better. No serious adverse event appears in any of the published studies, which are small and mostly by the manufacturer's advisors, so absence of reports is not proof of absence. Some US consent forms list localised overheating, a heavier period in the days after treatment, and panniculitis, an inflammation of the fat; we found no published case report of the last.

The risks that matter are the ones on the exclusion list: an electronic implant or metal near the applicator, a hernia the contractions could worsen, and pregnancy. Not everyone is suitable: the exclusion list and the skin type limits above apply to everyone.

How to choose an EMsculpt NEO clinic in London

  • Ask which machine. EMsculpt NEO, not the original EMsculpt sold under the same name, and a genuine BTL device with the clinic listed by BTL as a provider; ours is listed under Chelsea Clinic.
  • Ask who places the applicator and who supervises. Our sessions are run by trained clinicians in a doctor-led team, and this guide is reviewed by Dr Marie Duggan, GMC 7488383.
  • Ask for the plan per area, in sessions, in writing. A price for "a session" with no course is not a plan.
  • Ask about the exclusions before you sign. Implants, metal, hernia, pregnancy.
  • Ask for the clinic's own photographs, dated, with the number of sessions and the weeks since the last.
  • Treat percentages as averages. The 30% and 25% figures are the manufacturer's averages from its own advisors' studies, not a result for you.

Aftercare

There is little you need to do. Drink normally, keep training, and expect the muscle to feel worked for a day or two; in our experience an ordinary painkiller is enough if it aches. Keep your weight stable through the three months, because the fat result is measured against it. Come to the free review at three months, and tell us before then if the area is painful rather than sore, or if the skin blisters or changes colour.

Frequently asked questions

How much does EMsculpt NEO cost in London?

We price EMsculpt NEO from a starting price per session, per area and per course, confirmed in writing at a free consultation, and we price match any comparable written quote. Finance through Humm is available, subject to status. Consultations and follow-up reviews are free.

How much do 4 sessions of EMsculpt NEO cost?

We price EMsculpt NEO from a starting price per session, per area and per course of four, confirmed in writing at a free consultation, and we price match any comparable written quote. Consultations and follow-up reviews are free.

Is EMsculpt NEO really worth it?

If you have a soft layer over a muscle you already use, a body mass index under about 30 and realistic expectations, it can be: the trials show the area firmer and the fat layer a few millimetres thinner at three months, held at six. If you want to lose weight, or your bulge can be pinched, a different approach, such as diet, fat freezing or surgery, may suit you better, and we will say which at the consultation.

Is 3 sessions of EMsculpt enough?

The sham-controlled trial and the manufacturer's FDA abdomen study used three sessions and measured a result, so three is a minimum rather than nothing. The flank, arm and buttock studies used four, and so do we; in our experience the fourth is where the change becomes visible. One session is a demonstration.

What is EMsculpt NEO, in one sentence?

A 30-minute treatment that stimulates the nerves of a muscle group with thousands of electromagnetic pulses while radiofrequency warms the fat over it, to firm the muscle and thin the fat layer in a treated area.

How long do the results last?

Held at six months in the sham-controlled trial, and for the muscle-only version a 21-patient uncontrolled study by manufacturer advisors measured the changes about a year later: fat about 15% thinner and muscle about 19% thicker than at baseline. In our experience the muscle fades like any training if you stop using it, and the fat holds while your weight holds.

Does EMsculpt NEO hurt?

It is intense rather than painful for most people: strong contractions in waves and warmth from the radiofrequency, set to a level you can hold. In the manufacturer's FDA study 93% reported none to minimal discomfort. Expect muscle soreness for a day or two afterwards.

Is there any downtime?

None. You walk out and go back to your day; the skin is pink for an hour and the muscle feels worked for a day or two. The trials report no recovery period.

Which areas can be treated?

For fat and muscle together: the abdomen, the flanks, the thighs and the upper arms. For muscle toning only: the buttocks and the calves. The chin, the face and the neck are not EMsculpt NEO areas.

Does EMsculpt NEO work on the stomach after pregnancy?

It is a common request. Two small uncontrolled studies of the original EMsculpt, one in 10 postpartum women, found the gap between the abdominal muscles narrowed by about 10% to 23%, and an independent review found the change across studies was not statistically significant. We wait until you have recovered from the birth, check for a hernia first, and do not treat while you are pregnant or, on most clinic guidance, breastfeeding.

Does EMsculpt NEO work for men?

Yes. In our experience men ask for the abdomen and the flanks most. The sham-controlled trial enrolled men and women and found no link between sex and result.

What is the difference between EMsculpt and EMsculpt NEO?

The original EMsculpt stimulates the muscle only and has no fat-reduction clearance. EMsculpt NEO adds synchronised radiofrequency that warms the fat, and in the manufacturer's comparison the fat change rises from about 19% to about 30% and the muscle change from about 16% to about 25%. If a clinic's price seems low, ask which one it runs.

Is EMsculpt NEO FDA approved?

It is FDA-cleared in the US, which is a different thing from approved: for fat reduction and circumference reduction of the abdomen (2019), the thighs (2021), the upper arms (2022) and the flanks (2023), and for muscle toning of the abdomen, buttocks, thighs, calves and arms (2018 and 2019). Buttocks and calves have no fat indication.

Can I have EMsculpt NEO with a coil, a metal implant or a hernia?

A copper coil, metal in or near the area and any electronic implant are on the exclusion lists, and a hernia in the area means we do not treat over it. Most hormonal coils contain little or no metal (some have a small silver ring), so we check the manufacturer's guidance for your device at the consultation rather than assume.

EMsculpt NEO or CoolSculpting?

EMsculpt NEO for a soft layer over a muscle you want firmer; CoolSculpting for a fold you can pinch. They do different things, we found no trial that compares them, and some people have both. We offer both and we will tell you which fits.
